Strengthening Primary and Community Care for Australia's Veterans Most at Risk

Australian Medicare Local Alliance (AML Alliance) (formerly Australian General Practice Network) in conjunction with the Australian Practice Nurses Association (APNA), Flinders University and Baker IDI Heart and Diabetes Institute, have been engaged by the Department of Veterans’ Affairs (DVA) to provide training and resources in chronic disease management for General Practitioners (GPs), Practice Nurses, Community Nurses and Aboriginal Health Workers who will be implementing DVA’s new Coordinated Veterans’ Care Program.

The Australian Medicare Local Alliance’s role is to provide training in chronic disease management to GPs and nurses implementing the program, as well as producing a range of online program resources such as templates and guidelines. Training will be provided in four modules. All modules will be available online.  For those who have difficulty with internet access, Modules One and Two will also be available as a  Manual, all Modules are available in DVD format.  An optional workshop to supplement the online learning for Module  Two will be offered in all capital cities and in some regional areas which have a large population of veterans eligible for the CVC Program.
